Piles is swollen veins and surrounding tissue just at the opening of the anus. When the person passes stool, this growth pops out and is painful at times. Piles and Haemorrhoids are one and the same thing. Fissure is a cut or open wound in the skin of the rectal canal just inside the opening of the anus. Piles can be painless or could be bleeding piles. Some patients would see blood while passing the stool which is known as bleeding piles. Piles can be classified into 4 grades. In grade one there is small inflammations inside the lining of the anus which is not visible and normally doesn't give any problems. ln grade two the piles become slightly bigger but still remain inside the anus. They may pop out while passing the stool but go back without any effort. In grade three the piles hang outside the rectum and there is a feeling of something present at the entrance of the anus, though they can be pushed inside by hand. Grade four piles cannot be inserted back into the rectum and need immediate medical attention. There is no specific test for diagnosing piles and the surgeon can get a good idea by clinical examination. For internal piles a device called proctoscope is used to get a clear view of the piles and their grade. Piles are mostly caused by problems in the digestive system. Chronic constipation and putting pressure while passing the stool is one of the most common causes for development of piles. The treatment also lies in dietary modifications. Eating lots of fibre in the form of salads and fruits and drinking more water helps in most of the cases. Losing body weight, exercising regularly also helps in improving the excretary system thereby reducing the strain while passing of the stools. Some medicines and laxatives both local and oral also help in alleviating the pain and discomfort. However for grade 3-4 and bleeding piles, one has to get an interventional treatment like banding, sclerotherapy or surgery.
You must have heard the old treatment of Bawaseer as tying the hair of horse at the base of the pile. Similar thing is done by the doctors who place an elastic band at the base of the piles while cuts the blood supply and the haemorrhoid falls off. In Sclerotherapy, a medicine is injected into the haemorrhoid which makes it shrink and ultimately disappear. These treatments work on piles which are grade 1-3 but not on grade 4 piles. For this one has to go for surgery. In surgery there are three options - open surgery which has always been done by almost all the surgeons is the most common. Some of the newer surgeons are adept at doing Stapler Haemorrhoidectomy in which there is much less blood loss and the recovery is much faster. The cost of stapler is close to Rs. 20,000 which makes this option costlier. No many surgeons are able to do Stapler surgery and that is the reason that it has not become very popular amongst the patients. In open surgery, the surgeon will cut the piles which will result in some blood loss and then put stitches at the incision. Since the area is inside the rectum from where stool continues to pass, these stitches take time to heal. In stapler surgery there is no cutting of the piles and the surgeon uses Staplers to tie the hanging haemorrhoid inside the rectum. This is as effective as cutting but without the discomfort of stitches and has much less healing time. The person can get back to work much quicker. The third option is Laser Surgery for Piles in which the surgeon uses a Laser machine to coagulate and stop the flow of arterial blood to the haemorrhoids. The time taken for the procedure is much less, the procedure can be done in daycare and the recovery period is much less. Our experience and studies also show that Laser Haemorrhoidectomy has greater advantages in piles which are upto grade 3. There is much less post-operative pain in Laser Surgery as compared to the open surgery in such patients.
