Knee Joint is the biggest and the most complex joint in our body. It joins the Femur - Thigh bone and the Tibia - Shin bone. The joint provides flexibility in movements like walking and sitting. The joint itself has an upper part, a lower part and the knee cap. Between the upper and the lower parts of the knee joint is a soft tissue which is called cartilege. One of the biggest reason for knee joint pain is osteoarthritis which is compounded by obesity and lack of exercise resulting in weak lower body muscles. The leg muscles which support the knee joint are the quadriceps - front thigh muscles and the hamstrings - back thigh muscles. People who perform less physical activity tend to have weak thigh muscles. Add to this excessive body weight and this is a perfect recipe for pain in the knee joint. In osteoarthritis the density of the bones is reduced which weakens the knee joint. This results in reduction of the space between the upper and the lower part of the knee joint and destroys the cartilege. When the cartilege is diminished, the upper and lower part of the knee joint start brushing against each while walking which causes pain and knocking sounds in the knee. One option is to put synovial fluid in the knee joint through an injection. This helps in reducing the pain for about 8-10 months. The cost of the injection is close to Rs. 20,000 and therefore it is not an option for all. If body weight can be reduced by about 5-10 kg then a lot of patients will get relief from pain and then specific exercises for strengthening the leg muscles can avoid the need for knee replacement surgery. However if the patient is not able to lose weight or experiences pain despite losing weight then surgery is the best option for long term pain relief. The best time to get the surgery is when the patient is still walking and is not wheechair bound or bed ridden.
Most of the patients are highly scared of getting the knee replacement surgery and keep tolerating the pain until the situation becomes pretty bad and the patient is either on the wheelchair or on the bed. If the patient comes early then even partial knee replacement can give tremendous benefits to the patient. When the patient delays the surgery then Total Knee Replacement is the only option left. Even in the Total Knee Replacement Surgery, the entire knee joint is not replaced as the name might suggest. Only the top of the knee joint is changed. The surgeon will make an incision on the knee joint and remove the upper portion of the knee joint which has degraded due to osteoarthritis. This is replaced with the knee implant and the implant is fixed with the Tibia and the Femur bone using screws. The surgeon makes a perfect alignment of the knee joint with the hip joint and then closes the incision using dissolvable stitches. After the surgery the patient is shifted to the post operative ward till the effect of the anaesthesia wears off. The patient is put on painkillers so that there is minimal post operative pain. By evening the patient is shifted to the room. Next day, the physiotherapy team starts mobiliization and the patient is made to stand on the legs with the help of a walker. The patient is taken to the toilet with assistance on a walker. From the 3rd day, walking with the help of a walker is started. The patient is discharged from the hospital on the 5th day and home physiotherapy is started. The patient is called to the hospital on the 15th day after the surgery to examine. The physiotherapy continues for one month after which the patient is able to do everything on its own..
